Provides education in group and individual settings for patients with Type 1, Type 2, Gestational Diabetes, Pre Diabetes and Metabolic Syndrome. Provides education tailored to special learning needs. Identifies the learning needs of patients and or families and assesses readiness to learn. Evaluates the patients understanding of information taught, provides reinforcement and follow up as necessary. Documents teaching activities and each patients response completely and accurately. Assists to develop and refine lecture and teaching materials. Assists with community education programs and ADA advisory meetings, activities as needed. Assists with keeping teaching sites supplied. Participates in quality and performance improvement activities. Follow OSHA, Infection Control and HIPPA guidelines. Maintains professional credentials and CME credits to maintain CDE, ADA eligibility. Strives to improve knowledge base and skills. Works closely with other DSME educators and Program Coordinator to ensure high quality education and a safe environment for patients, families and staff.
3 plus years of experience RN - Registered Nurse CDE preferred. Has required CEUs in diabetes. Registered dietitian with current license to practice in the State of Ohio
