What You’ll Do:
- Provide physician-prescribed physical therapy in patients’ homes
- Conduct initial assessments, develop care plans, and reassess every 30 days
- Perform skilled therapy procedures and teach patients and families home exercises and activities of daily living
- Collaborate with physicians, families, and other home health personnel to optimize outcomes
- Document visits, OASIS assessments, progress notes, and discharge summaries accurately and timely
- Participate in in-service programs, patient care conferences, and Quality Assessment & Performance Improvement activities
- Supervise Physical Therapy Assistants and Home Health Aides as assigned
Qualifications:
- Graduate of an accredited Physical Therapy program approved by CAPTE, APTA, or relevant governing bodies
- Licensed in Ohio (or applicable practicing state)
- Minimum of 2 years’ experience preferred
- Strong initiative, independent judgment, and commitment to patient-centered care
- Alignment with the philosophy and goals of our agency
Working Environment:
- Travel to patient homes and work in-office as needed
- Physical activity including bending, lifting, and standing for extended periods may be required
