Duties and Responsibilities
- Provide direct nursing care as dictated by the patient’s care plan, including management of pain and symptoms to ensure patient comfort and quality of life.
- Monitor patient condition regularly and adjust care as circumstances warrant, ensuring seamless communication and documentation.
- Assist with the implementation of physician orders while ensuring compliance with all healthcare regulations and standards.
- Engage and educate families in patient care processes, offering emotional support and guidance throughout their loved one’s end-of-life journey.
- Participate in the development and review of patient care plans in collaboration with interdisciplinary team members.
- Administer medications accurately and effectively with close attention to patient reactions and possible side effects.
- Coordinate with healthcare providers and hospice care teams to evaluate and manage hospice care treatments and interventions.
- Document patient data precisely and comprehensively for utilization in ongoing care assessment and compliance with legal requirements.
- Be responsive to the emotional, spiritual, and practical needs of patients and families, ensuring that care provided maintains the dignity of all parties.
- Attend regular team meetings to discuss and align on patient care priorities, updates, and required adjustments to care plans.
- Maintain a clean, safe, and organized patient environment at all times in adherence to safety protocols and standards.
Requirements
- Current and valid LPN license in the state of employment.
- Proven experience as an LPN, preferably in a hospice or palliative care setting.
- Strong understanding of the hospice care philosophy and willingness to provide end-of-life care that respects the dignity of the individuals and families served.
- Demonstratable skills in administering medications and understanding of potential side effects.
- Excellent observational and critical thinking skills; able to make informed decisions in a compassionate and ethical manner.
- Proficient in maintaining detailed and accurate patient records and documentation.
- Exceptional communication and interpersonal skills, capable of handling sensitive situations with empathy and calm.
- Ability to work both independently and as part of a diverse interdisciplinary team.
- Commitment to ongoing professional development and adherence to applicable healthcare guidelines and regulations.
- Physical ability to perform assigned duties, including the ability to move or lift patients and equipment.
