Position Summary:
Meridian HealthCare is seeking a Behavioral Health Manager - Housing to support the delivery of high-quality, patient-centered services within our Housing program. This program serves individuals experiencing mental illness, co-occurring disorders, and histories of homelessness.

The Behavioral Health Manager - Housing provides both direct care and clinical leadership—mentoring Therapist Assistants, facilitating staff training, and ensuring compliance with all housing and clinical documentation standards. This position plays a key role in promoting housing stability, wellness, and community integration through coordinated, trauma-informed, and housing-first approaches.


Key Responsibilities:

  • Carry a caseload of patients and deliver comprehensive, patient-centered care to support housing retention, mental health stability, and overall wellness.
  • Conduct psychosocial, housing retention, and functional needs assessments at intake and ongoing intervals using approved tools and timelines.
  • Mentor and guide Therapist Assistants through case consultation, resource support, and ongoing performance feedback.
  • Support onboarding and training of new staff through shadowing, modeling, and documentation coaching.
  • Facilitate or co-lead team meetings and interdisciplinary case reviews with clinical, medical, and housing staff.
  • Coordinate care with external providers, including behavioral health, primary care, and community-based services.
  • Participate in incident reporting, risk management, and emergency response as needed.
  • Promote and uphold trauma-informed, harm-reduction, and housing-first principles in all aspects of service delivery.

Qualifications:

  • Master’s degree in Social Work, Counseling, or related field.
  • Active LISW or LPCC license required.
  • Minimum of 3 years of experience in case management, supportive housing, or community mental health.
  • Demonstrated experience conducting behavioral health or housing-related assessments.
  • Knowledge of Housing First principles, Medicaid documentation standards, and social determinants of health.
  • Experience working with individuals with serious mental illness, substance use disorders, and homelessness.
  • Strong written and verbal communication skills, leadership ability, and teamwork orientation.
  • Familiarity with local housing systems, tenant rights, and supportive service networks.
  • Proficiency with electronic health records (EHR) and Microsoft Office Suite.
  • Valid driver’s license and ability to travel locally for home visits and appointments.
