Compassionate Hands Home Care is looking for a Home Health Registered Nurse that is willing to:
1. Supervises and Train Personal Care Aides
2. Make initial visits to the patient and every 60 days thereafter
3. Demonstrate competency in all skills required for the agency.
4. Evaluate and re- evaluates the needs of the patient; initiates , develops, implements and makes necessary revision to the patient's plan of care. Assesses the patient's continual care needs. Addresses all problems identified in the comprehensive assessment in the plan of care or provides evidence of a rationale for not doing so.
5. Initiate diagnostic, preventive and rehabilitative nursing procedures to the patients care and safety. Make referrals to other members on her care team by the needs of the patient or document by not doing so.
6. Observes signs and symptoms and reports changes in the patient's condition to the patients care team as often as needed.
7. Teaches, supervises and counsels patients and family regarding home health nursing procedures and other care needs as appropriate to the patients conditions. Utilize educational material as appropriate.
8. Coordinates the plan of care and maintains continuity of patient care by acting as liaison with the patient care team that's assigned to the patient.
9. Participate in all aspects of care including and ongoing development of the plan of care. Initiates patients' plan of care whenever needed to ensure good coordination of care.
10. Delegates tasks for which they believe the PCA has the knowledge and skills to perform, taking into consideration training, cultural competence, experience and agency policies and procedures.
11. Supervises and teaches PCA, per policy; assesses patients' need for unskilled care and revises plan of care as appropriate.

CPR and First aid Certification

BCI and FBI ( if you have not lived in Ohio for 5 yrs.) initial and every 5 years thereafter

Will need to live near the Lucas County Area and willing to travel
