This position has primary responsibility for the Service Coordination program meeting specific goals, collecting required data, ensure reports are timely and enter data into appropriate systems. The Team Leader is the liaison between the service coordination program and the community. Where applicable, ensures fidelity to evidence-based practice models.


The Service Coordinator Team Leader leads and coordinates the individualized, strength-based youth and family planning process for youth with complex needs. This role brings together youth, families, natural support, and service providers to create a team-based care plan that promotes stability, well-being, and success across life domains.

  • Screens referrals for service coordination and assigns cases to other service providers. Communicates
  • with referral source, family and DJFS the decision on the referral
  • Tracks all referrals, date of referral, disposition of referral and ensures timely contact is made with the
  • family following program guidelines
  • Takes lead in scheduling IRC review meetings in accordance with family team meeting schedules.
  • Supports high fidelity wraparound services are being delivered.
  • Monitors and calculates data collected for the program to ensure it is always current and complete.
  • Submit monthly and quarterly reports as required to DJFS staff, FCFC and other key stakeholders
  • Engage youth and families in culturally responsive, trauma-informed planning that values their voice and choice.
  • Facilitate Child and Family Team (CFT) meetings involving families, service providers, school staff, and community members based on families’ requests.
  • Develop and manage individualized wraparound care plans with measurable goals and outcomes.
  • Coordinate services across systems (mental health, education, juvenile justice, social services, etc.) to reduce duplication and improve outcomes.
  • Identify and mobilize natural supports (extended family, mentors, neighbors, etc.) to strengthen the youth’s support network.

Qualified candidates must have:

  • Bachelor’s or Associate's degree in social work, Psychology, Counseling, or related field (Bachelor’s preferred) -OR-
  • Three years’ experience in working with youth with an emotional disturbance or a
  • Familiarity with wraparound principles, strength-based approaches, and family-driven care.
  • Strong facilitation, communication, and problem-solving skills.
  • Demonstrated ability to engage resistant or underserved families.
  • Ability to work effectively with diverse populations and across service systems.
  • Proficiency in case management documentation and outcome tracking.
  • Valid OH driver’s license and reliable transportation and the ability to be covered under the MHS liability policy.

MHS provides CPR/First Aid and NVCI (CPI Blue) for all new staff, along with on-going education and on-the-job training opportunities.

All MHS candidates are required to have an Ohio BCI check (FBI check required if you have lived in Ohio for less than 5 years, or for working with children), 5-panel drug screen and Residential candidates must have a 2 step TB (or proof of prior TB) upon conditional offer of employment.
