Job Responsibilities:

Job Qualifications:

Direct Patient Care &amp; Case Management

<li>Provide skilled nursing visits to approximately 20 patients per month</li>
<li>Conduct comprehensive assessments and develop individualized care plans</li>
<li>Manage complex, high-acuity cases requiring specialized nursing skills</li>
<li>Coordinate interdisciplinary care with physicians, therapists, and other healthcare providers</li>
<li>Complete OASIS assessments, care plan updates, and clinical documentation</li>
<li>Serve as clinical resource for challenging patient situations</li>
</ul>
Strategic Leadership 
<ul>
<li>Participate in organizational strategic planning and clinical decision-making</li>
<li>Analyze market trends and competitive landscape in home health services</li>
<li>Develop metrics and reporting systems for clinical quality and partnership success</li>

QUALIFICATIONSRequired:
<ul>
<li>Master's degree in Nursing (MSN) preferred; Bachelor's degree in Nursing (BSN) minimum</li>
<li>Current Ohio Registered Nurse license in good standing</li>
<li>Minimum 5 years nursing experience with acute care and case management background</li>
<li>Home health care experience strongly preferred</li>
<li>BLS certification required</li>
<li>Valid driver's license with reliable transportation and clean driving record</li>
