Responsibilities
The individual must be detail oriented, have the ability to lead and deliver high quality services in a person-centered environment. This position coordinates the development, completion and transmission of assessments in accordance with the regulatory requirements and reviews and manages reimbursement. The MDS Coordinator is responsible to case manage care plans, including rehabilitative programs. Qualifications

PHYSICAL REQUIREMENTS

<li>This role requires a variety of physical activities to effectively perform essential job functions. The position involves frequent walking (75%), sitting (50%), and standing (50%), with regular bending, stooping, and reaching (25–50%). Employees must be able to lift, carry, push, and pull items up to 25 lbs. Strong fine motor skills and full use of hands are essential, as the role demands constant grasping, writing/typing, and use of technology. Visual and auditory acuity—including color, depth, peripheral vision, and the ability to adjust focus—is required 100% of the time. Occasional driving or climbing may also be necessary. </li>

What you'll be working on:
<ul>
<li>Managing a patient panel with a broad array of patient needs; conducting a mix of acute, chronic, and well visits</li>
<li>Promote awareness of One Medical's clinical services and presence in Northeast Ohio, particularly among the clinical community of Northeast Ohio; participate in local marketing events</li>
<li>Treating patients in-office or in testing centers as well as conducting occasional tele-health visits</li>
<li>Continuous learning during weekly Clinical Rounds and through other modalities</li>
<li>Ongoing collaboration with in-office teammates via daily huddles, as well as with virtual clinical teams </li>
<li>Utilization of your specific clinical training and opportunities to perform in-office procedures </li>
<li>Supervising one or more NP or PA colleagues</li>
</ul>
Education, licenses, and experiences required for this role:
<ul>
<li>Practiced the last 3 years in an outpatient primary care setting with a track record of strong patient outcomes and engagement</li>
<li>Board certified in Family Medicine </li>
<li>State licensed in Ohio, obtained before your One Medical start date</li>
</ul>
