Responsibilities

<li>Provides personal care</li>
<li>With respect to patient rights, ensures all care is provided with respect and dignity to patients and their families</li>
<li>Demonstrates and maintains a positive, professional manner</li>
<li>Keeps a record of hospice Certified Nursing Assistant - CNA/STNA plan of care for each patient assigned </li>
<li>Observes, records, and reports the patient's care, symptoms, emotional and physical status </li>
<li>Watches, observes, and reports any physical and gross behavioral changes in the patient's condition or family situation to the RN</li>
<li>Assists in feeding the patient as needed</li>

<li>Minimum of one (1) year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.</li>

The Role:
We are looking for a compassionate Licensed Therapist to support our mission in democratizing access to high-quality mental health care! Cerebral takes a holistic and comprehensive approach when providing care for our patients. Our telemedicine therapists collaborate with prescribers as needed, to support clients during their mental health journey. This therapist role provides direct care for a panel of clients and allows for flexibility when client sessions can be scheduled. You can see clients during traditional business hours, evenings, or on weekends.
This position is a 1099 independent contract role. 
Current state license requirements include: Alabama, Georgia, Indiana, Iowa, Maryland, Nebraska, Ohio, South Dakota, Texas, Virginia, Washington, and/or West Virginia.
If licensed in Iowa, Nebraska, or South Dakota, you must also reside in that designated state due to current payor requirements.
Who you are:

<li>Independently Licensed Therapist in the United States for any of the following license structures: Professional Counseling, Marriage and Family Therapy, or Clinical Social Work</li>
