Responsibilities

Therapist Summary:
Daybreak seeks a Licensed Professional (LSW/LPC) with dynamic energy and demonstrated experience empowering high-risk youth to achieve their optimal well-being. Provides mental health assessments, treatment planning, individual/group psychotherapy, and psychotherapy for crisis for minors and young adults referred to all Daybreak programs - Shelter (s), InReach, Drop-In, David's Place, Housing, and Employment.
Therapist Responsibilities and Duties:
Provide Mental Health Assessment and crisis intervention assessment/plan for Daybreak clients.
Develops and oversees implementation of the individualized services plans (ISP)/ treatment plans in conjunction with the client and assigned case manager.
Provides mobile services to Daybreak clients including; serving Daybreak clients in the Adult Shelters in the Miami Valley.
Provide individual counseling for Daybreak's LGBTQ+ clients, identifying triggers that would put them at risk or result in functional impairments by utilizing evidence-based practices to advance progress toward treatment goals.
Provide as a resource on LGBTQ+ population, offering competent skills in this area to meet LGBTQ+ needs.
Provides individual and group counseling for clients based upon their ISP goals and objectives.
Serves as a resource on MH/SUD Clinical Issues.
Refers clients to community resources and other mental health organizations in collaboration with their case managers and program directors/managers.
Maintains records on all clients in the manner prescribed by agency procedure and in accordance with the requirements of funders, monitors and professional standards.

Meets 40% productivity standard established by the agency and outlined in writing in supervision.

Implements and completes ISP reviews and performs follow-up to determine quantity and type of service provided to the client.

Background Check

Pre-employment Testing
The following pre-employment tests will be conducted at Daybreak's expense. Tests include:

<li>A tuberculin skin test or chest x-ray (by a physician of Daybreak's choosing) </li>
<li>A urine drug screening (by a testing facility of Daybreak's choosing)</li>
</ul>
Should the pre-employment reviews indicate that the prospective employee does not meet the employment requirements, Daybreak has the option of either not hiring the person or postponing employment until the condition prohibiting employment has been removed.
If the background or pre-employment checks contain information that the applicant has committed, been convicted of, or pleaded guilty to any criminal offense, unless the charge was a minor misdemeanor not involving violence or a sexual offense at least 5 years old, or if the applicant refuses to submit fingerprints for a criminal background check, the conditional offer of employment will be withdrawn immediately.
Pursuant to SB 38 (child-serving agency), if it is determined that the applicant has committed one of the disqualifying events listed on the attached Ohio Revised Code Disqualifier sheet may prevent employment at Daybreak.

Under the general supervision of the Director of Service and Support Administration, this position is responsible for the development of a person-centered plan for individuals with developmental disabilities that promotes self-determination. Assists individuals and their families/guardians to explore and obtain services and supports from a variety of sources. Assures that services delivered follow Federal, State, local rules, regulations, statutes, standards, as well as Board policy, regulations and procedures and related Operating Plans. An Associates degree in a related field and (3) years' experience in coordinating services in DD or related human services required, Bachelors degree preferred. You must possess or be willing to obtain and maintain certification from Ohio Department of Developmental Disabilities; applicants should have a working knowledge of persons with disabilities. Individual must be able to pass criminal background checks as required by the Ohio Revise Code; have a valid Ohio driver's license, current motor vehicle insurance with a clean driving record. Must be able to pass a pre-employment physical (including a TB screening) and drug screen. This position has a 180-day probationary period following first day of full-time employment.
