Providers see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs. 
 
Dedicated schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.
 
Specific duties of the role include, but are not limited to:
 
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and responses to treatment
- adjust treatment plans as necessary
- perform minor procedures and wound care as required (and as settings permit)
- educate patients and families on health management, disease prevention, etc.

We are seeking a highly skilled and compassionate resident montiors to join our team. The ideal candidate will work in a collaborative team environment, providing therapeutic interventions to individuals struggling with behavioral health challenges. As a resident monitor, you will have the opportunity to help clients overcome barriers, provide solution-focused interventions, and make a meaningful impact by saving lives.
Responsibilities:
<ul>
<li>Provide a safe, secure, structured milieu</li>
<li>Act as a non-clinical resource for clients, for example; provide crisis intervention, be an active listener to client, answer non-clinical questions for client</li>
<li>Maintain appropriate written documentation and written and/or verbal reports on client activity and behavior</li>
<li>Perform any required facilities maintenance and/or report issues to Supervisor</li>
<li>Perform administrative office tasks, as example: client chart reviews, answering phones, greeting guests, copying, and filing</li>
<li>Transport clients to offsite meetings/activities, as needed</li>
<li>Participate in monthly individual supervision</li>
<li>Participate in all required training programs</li>
<li>Facilitate required groups and structured activities</li>
<li>Participate in a 30-minute shift change, including giving or receiving verbal shift report and passing keys to incoming staff</li>
<li>Perform other duties as requested.</li>

Qualifications:

<li>Required to be 21 years old, high school graduate or equivalent</li>
<li>Take all steps and precautions possible to maintain and ensure client confidentiality</li>
<li>Maintain appropriate professional boundaries when interacting with clients and fellow co-workers</li>
<li>Maintain all necessary documentation as required with our policies and Procedures</li>

Pay: $65 - $68 an hr | Weekly Pay!

Nurse Practitioner Overview:
Correctional NP Needed in Lima, OH - First Shift, 40 hours per week. Pay Rate: $65.00 to 68.00 per hour depending on experience!
<ul>
<li>Searching for a family or adult-gerontology nurse practitioner licensed to work in Ohio with at least one year of working NP experience in primary care to provide on-site services full-time to the population at this correctional facility.</li>
<li>The schedule is weekdays only (no evenings, weekends, or holidays), and shift start/end times have some flexibility based on the needs of the facility. An on-site interview will be required. Preferred Shift is MON-FRI 8a-4p!</li>
<li>The assignment is projected to last approximately 3-6 months but may be extended based on continued need and mutual fit.</li>
