General Summary:
The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing serious mental illness and/or addiction challenges to ensure their needs are addressed. The Case Manager supports clients in the development of their short and long-term goals and acquisition of community resources using a person-centered approach. Services will be provided with respect to cultural preferences and in accordance with client’s rights. <li>Assesses client strengths and needs with the client and applicable team members, conducts comprehensive care coordination, and develops treatment plans. Monitors and evaluates client progress toward goal attainment. Reviews and modifies treatment plans as needed.</li>

Position Qualifications:

Job DescriptionRecovery Center of Ohio’s mission is to help men and women reclaim their lives from the disease of addiction to alcohol and other drugs and to reduce the incidence of addiction in future generations. We are an agency committed to quality gender-responsive, trauma-informed care to clients with substance use and co-occurring disorders. The Addiction Clinician provides critical alcohol and other drug counseling, assessment and referral services to clients in a manner that reflects the agency's mission. Essential Duties and Responsibilities
<ul>
<li>Conduct screening interviews to determine the need for a drug and alcohol assessment and for emergent care services in the specific areas of detoxification, psychiatric care, and intensive outpatient treatment; make referrals to appropriate health and human services agencies as needed based on the outcome of the screening interview</li>
<li>Conduct confidential, quality assessments, utilizing ASAM Placement criteria and the Addiction Severity Index to determine the appropriate level of care;</li>
<li>Use a strengths-based and trauma-informed approach to work collaboratively with clients and internal and external team members to help support clients in their treatment and recovery</li>
<li>Provide clinical consultation to referral resources, Mentors, family members, and others to help support clients in their recovery</li>
<li>Work closely with clients to engage them in the decision-making process and help them understand the benefits of assessment and accessing the appropriate level of care as indicated</li>
<li>Collect and review referral information upon assignment of clients from the referral source and consult with others as needed to support coordination of services and scheduling of client interviews</li>
<li>Coordinate with the Intake Coordinator as needed to contact clients to confirm appointment dates, times, and assessment locations</li>
<li>Collect information from collateral resources to aid in determining diagnosis and need for treatment</li>
<li>Conduct IOP groups Monday through Friday as directed</li>
<li>Remain in compliance with internal and external policies, procedures, regulations, and standards, including all of the agencies policies and procedures and Quality Assurance Plan, and State Licensing standards</li>
