Overview
Otterbein has an opportunity for an MDS Nurse. The MDS Coordinator is a critical member of the health care team responsible for all aspects of the MDS process, including care planning and meeting with families regarding the care of their loved one.
Shift: Monday through Friday with on call rotation
Responsibilities
The individual must be detail oriented, have the ability to lead and deliver high quality services in a person-centered environment. This position coordinates the development, completion and transmission of assessments in accordance with the regulatory requirements and reviews and manages reimbursement. The MDS Coordinator is responsible to case manage care plans, including rehabilitative programs. It is important that this individual demonstrate knowledge of clinical standards of practice, regulations and reimbursement governing long-term care and Medicare participation.

Accountability for Hospice Aide:
<ul>
<li>Assist in maintaining a clean, healthful, and safe environment.</li>
<li>Assist the patient with ambulation. Uses safe transfer and ambulation techniques.</li>
<li>Assist with nourishment and fluid needs including prepared meals, feeding patient, and offering fluids.</li>
<li>Reading and recording temperature, pulse, and respiration.</li>
<li>Maintains basic infection control procedures.</li>
<li>Adherence to the Hospice Aide care plan.</li>
<li>Gives simple emotional and psychological support to the patient and other members of the household.</li>
<li>Establishes a relationship with patient and family, which transmits trust and confidentiality.</li>

Responsibility for Hospice Aide:
<ul>
<li>Contacts the Clinical Manager to inform him/her of alternate schedule for patient visits during vacation, sick days, or personal days off. Reports any schedule change or inability to complete assignment as scheduled.</li>
<li>Accepts changes/additions to schedule during staffing difficulties when necessary. Accepts assignment in time of emergency, regardless of any location and/or any day of the week be it weekend/holidays.</li>
<li>Compliance with all applicable laws, regulatory requirements, standards of practice, accreditation standards, and policies and procedures.</li>
<li>Organizes daily visit schedule to minimize travel time and mileage. Maintains productivity according to work status.</li>
<li>Maintains OSHA and Infection control per policy.</li>
<li>Performs other duties as assigned</li>

General Working Conditions: This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. SUMMARY: To provide direct, high-quality customer service to clinic patients and visitors and maintain a professional and friendly atmosphere. To coordinate AxessPointe Community Health Center, Inc. (API) health center appointments, provide reception services for patients, maintain patient charts, and represent the health center in all patient interactions. API manages patient care using a team-based approach in our interactions with patients and working to achieve stated objectives and outcomes.

DUTIES AND RESPONSIBILITIES:

<li>Provide exemplary customer service too all patients on the phone and in the health centers. </li>
<li>Answer incoming calls and schedule appointments (both in-office and telehealth).</li>
<li>Complete patient registration, insurance verification, and process/scan/fax all necessary paperwork.</li>
<li>Collect and process co-payments at time of service, understand payment arrangement policy and assign accordingly. Complete daily reconciliation of funds collected.</li>
<li>Complete virtual ‘Telephone Encounters’ in current operating system and assign to appropriate staff.</li>
<li>Check patients in &amp; out in a timely manner and consult with clinical support and/or provider staff regarding walk-in patients. </li>
<li>Assist patients navigate insurance plans, process claims, and work to create payment plans when necessary.</li>
<li>Complete insurance eligibility process. </li>

Requirements:
MINIMUM REQUIREMENTS:
<ul>
<li>Minimum of a High School diploma or equivalent is required.</li>
<li>1-3 years’ experience working with underserved/vulnerable populations and working with community resource programs is preferred.</li>
<li>Essential Technical/Motor Skills: Able to operate computer, fax machine, printers, scanners, webcam and other office machines and equipment. </li>
<li>Ability to successfully complete all required and applicable federal and/or state consumer assistance training.</li>
<li>Interpersonal Skills: Work independently and as a team member and at times with difficult clients </li>
<li>Essential Mental Abilities: Ability to meet and interact in positive way with people. Ability to organize and multi-task. Communicate effectively and in a culturally appropriate manner. </li>
<li>Adhere to the guidelines &amp; principals of API and any of its satellite locations. </li>
<li>Must have excellent oral and written communication skills and advanced computer skills.</li>
