OBJECTIVE:
The Supported Living Coordinator is responsible for the coordination and delivery of homemaker personal care services for intellectually and/or developmentally disabled adults in a supported living environment. The Supported Living Coordinator plans, organizes, directs, coordinates and manages supports provided by the Direct Support Professionals and the Lead DSPs. The SLC works to enhance the attainment of the Individuals’ personal goals and objectives while maintaining required standards of the various regulatory agencies including ensuring health and safety of the Individuals.

PRIMARY RESPONSIBILITIES: (including, but not limited to)
• Supervises Direct Support Professionals and Lead DSPs – Advising H.R. as to performance for reviews, feedback, and corrective action if needed.
• Schedules and conducts monthly Lead DSP meetings.
• Attends/Schedules/Coordinates meetings for Behavior Support, OISPs, Team meetings and Staff meetings as needed.
• Monthly reviews of all site documentation including but not limited to: OISP service delivery documentation, financial documentation, medical appointment documentation and tracking, MAR’s, shift log notes, behavior data and forms ensuring health and safety.
• Completes monthly site visits and maintains communication with members of the Individual’s team at least monthly.
• Completes QA reviews monthly.
• Communicates with Adult Day Service Providers monthly.
• Reviews of Fire/Safety checklists and faxed to the FCBDD SSA within 5 days of completion.
• Follows all guidelines for UI/MUI reporting per the Ohio Revised Code.
• Works open shifts as needed.
• Completes LPS billing monthly and turns in by the 5th of each month.
• Provides back up support to the Lead Staff on-call
• Assists with planning and coordination of outings, social events, and holiday events.
• Assists with scheduling and ensuring Individuals are receiving services as outlined in the OISP.
• Monitors and reduces Over Time hours.

<li>Collaborates with Judson Teams: Dining Services Manager, EVS manager, RCM’s, Volunteer Services manager, Art therapy Coordinator, Transportation Coordinator to plan programs/events and trips on a weekly, monthly, annual basis. </li>

<li>Actively seeks input for program scheduling and enhancement from resident committees and incorporates into programming as appropriate.</li>
</ol>
<ol>
<li>Analyzes program/trip offerings in terms of participation levels, needs/desires of residents, cost/benefit and staff resources and use the data to take the appropriate steps to enhance or modify programs/trips.</li>
</ol>
<ol>
<li>Tracks and coordinates program expenditures and adheres to established budgets. Process AP Vouchers/Avid to assure that providers of service are paid in a timely fashion.</li>
</ol>
<ol>
<li>Actively participates in meetings as required, including but not limited to Daily Stand – Up &amp; manager meetings, Resident committees, support meetings in order to facilitate communication, knowledge and support of all staffs’ effort to serve the residents.</li>
</ol>
6a. Audit and review with team Point Click Care records, (MDS), progress notes, service plans and support care conferences in each area of care as needed.

<li> Utilizes computer (Microsoft Office and other applications) to create program calendar, schedules, trip, flyers, mailings, inserts and other documents. </li>
</ol>
<ol>
<li>Able to work well with residents in all levels of care and residents with a variety of functional/cognitive capabilities. </li>
</ol>
<ol>
<li>Order food, beverages and supplies for resident events, including socials and resident birthday celebrations. Ensures that necessary food, beverages and supplies are stocked and available for these program events.</li>
</ol>
<ol>
<li>Manages team folders on S drive and distribution lists to send newsletter/calendars weekly.</li>
</ol>
<ol>
<li>Operate Audio Visual equipment needed for programs for Community Life events.</li>
</ol>
Miscellaneous: Maintains a record of acceptable attendance and punctuality. Observes all company rules and safety procedures. Other duties as assigned.

Minimum Qualifications, Education and Certifications: This position requires a minimum of a Bachelor’s degree in a recreation, social service or human services related field or at least 4 years experience working with older adults in a CCRC, long term care facility or community based setting. Computer literacy required, including strong skills in Excel, Word, PowerPoint, Publisher and Outlook with the ability and willingness to learn new applications and programs as needed.
Physical Requirements: The physical demands described here are representative of those that must be met by an associate to successfully perform the essential functions of this job:
Physical demands may include standing for extended periods, walking, assisting residents with wheelchairs, walkers, canes, etc., bending, stooping, sitting for 1-2 hours at a time, lifting up to 25 lbs, climbing stairs and repetitive motion. Ability to communicate verbally, including articulation and enunciation to be heard and understood and to perceive and understand verbal and written communications is required as is the ability to communicate in writing, use personal computers, telephones and other standard office equipment and to operate a motor vehicle in the course of business.
Working Conditions: Office environment and outdoor environments as needed to accompany residents on variety of trips and to external programs. This position requires the flexibility to provide daytime, evening, weekend and holiday hours.
